Mental Health Survey

Have you used NHS mental health services in Carmarthenshire, Ceredigion or Pembrokeshire recently?

We’d like to hear from people who have accessed care close to home (GP or community mental health support) or spent time in hospital or a mental health unit. We’re also keen to hear about people’s experiences around care and treatment plans.

Are you a carer or family member who supports someone with mental health issues? Hywel Dda Community Health Council is a patient watchdog body and we need to know about your views on NHS services, too.

The information you provide is totally confidential and we will not identify you (or the person you support if you’re a carer). We are working with West Wales Action on Mental Health who helped to develop this survey.

When we have gathered your views, we will look at all responses and speak directly with Hywel Dda Health Board with our findings. We will also write a report that you will be able to see on our website or you can request a copy to be sent to you when it is available.
